2024-25 Athletic Department Paperwork
All athletic forms for the 24-25 school year are to be completed online in PowerSchool. Only the form for a physical will be available on paper.
Only athletes who have a current physical on file in the Athletic Office and have completed all forms online, will be allowed to tryout, practice and participate.

This is the form that student-athletes and parents should read prior to signing electronically acknowledging reviewing the Athlete-Parent Agreement.

Travel Release Form
Student-Athletes that are unable to be transported by district-sponsored vehicles must fill out a travel release form for each event that will require a different form of transportation.
